What Is Community Care

Community care is about enabling you to be an active member of your community. It’s about supporting you to continue with the activities you love, or to engage with something new. 


We normally provide community based support on an hourly basis. This support can be 1:1 or as part of a group. Our groups are very popular and they’re a great way to grow your social circle, but there’s never any pressure to join a group and if you’d prefer to be supported in the community on a 1:1 basis, we will be very happy to facilitate you. Quite often people mix their support between 1:1 and group activities depending on how they’re feeling, and which groups they want to take part in.

 

And of course we’re very keen to support you with your existing hobbies and interests, so if you’d like to attend a group organised by someone else and you want us to come along with you, we can definitely arrange this.

Our group activities take place under the banner of The Enable Project, which is a ground-breaking initiative that promotes real inclusion for people with learning disabilities, Autism spectrum disorder (ASD), physical disabilities and mental health conditions. We provide safe and friendly support to people of all ages, as participants take part in daily activities to develop their skills and confidence during the transition from youth to adulthood, and beyond

How Does Community Care Work

Community care can be funded privately, through Direct Payments or a commissioned package.


If you’re not currently in receipt of any financial support, we’d recommend contacting your local council to find out what you might be entitled to. 

We provide door to door transport which is free of charge to all participants. This includes a pickup and drop off service from and to your home address, as well as transport between activities.

GROUP RATIOS
We offer 1:1 community care, but in addition we also provide group support. We recognise that everyone is an individual, and we always work closely with participants and their loved ones to determine what level of support they need, and in which setting they'd feel most comfortable.

We normally offer support on a 3:1, 2:1 and 1:1 basis, depending on your needs and wishes. Some people join other participants as part of a group for some sessions, and have 1:1 support during other sessions. Our community care services really do offer a bespoke approach that is completely tailored to your needs, so don't be afraid to get in touch for a chat - we're very friendly, and very flexible.

 

We are also fully regulated by the CQC, so we can provide personal care if required.

We always carry out a full assessment of each participant’s needs in order to ensure everyone receives the right level of support whilst out and about with our activity co-ordinators and support staff

Who Is Community Care For

Community support is for everyone, whether you’re under 18, over 18, have a disability, mental health condition, or need some physical or emotional support in getting out and about. We don’t discriminate and we assess all clients individually to establish what kind of support you’d like, and how often you’d like it.

We work with you to set any goals you might have your sights on, so our team know what you’re working towards. It might be that you just want to regain some of your independence, or perhaps you’re taking the first steps into adulthood and you’d like to try some new things.

 

Whatever your needs, whatever your goals, we’re here to help.
